What You Need to Know About the Program
How It Works
Applicants may apply to receive a Kidney “Stuff” supplement donation through Operation Health Transformation (OHT) in just a few minutes with our easy-to-use and secure online form below. We require complete responses and a mandatory picture submission via email should you be selected.
Who Can Apply
The program is open to everyone in need. You must be a resident of the United States or Puerto Rico, and 18 years of age or older. We accept personal and nominated applications from individuals who genuinely need our products, are struggling with their overall kidney health, have been diagnosed with a form of kidney disease, or are on the path to dialysis.
